Urinary Tract Infections in Children ကေလးဆီးလမ္းပိုးဝင္ျခင္း

မဂၤလာနံနက္ခင္းပါဆရာ။ အသက္ ၄၆ရက္၊ မိန္းကေလး၊ ကြၽန္မရဲ႕သမီးေလးက ဆီးသြားခါနီးတိုင္း စူးစူးဝါးဝါးအသံနဲ႔ ေအာ္တတ္ပါတယ္ ။ ဆီးကေတာ့ တစ္ေန႔ကို ပ်မ္းမ်ွ အႀကိမ္၃၀ ထက္မနည္း သြားပါတယ္ဆရာ။ အခုခ်ိန္အထိ မိခင္ႏို႔တစ္မ်ိဳးတည္းကိုသာတိုက္ေကြၽးပါတယ္။ သူ႔အတြက္စိုးရိမ္ရပါသလား႐ွင့္ ေနာက္ၿပီး တစ္ေန႔ကို ၃ႀကိမ္ေလာက္ ႀကိဳ႕ထိုးတတ္ပါတယ္။ ကုသနည္း ႏွင့္ ေဆာင္ရန္ ၊ ေ႐ွာင္ရန္ နည္းလမ္းမ်ား ၫႊန္ျပေပးပါ႐ွင့္။ ေက်းဇူးတင္ပါသည္။ ဆရာက်န္းမာခ်မ္းသာပါေစ။

ကေလးဆီးလမ္းပိုးဝင္တာ အသင့္အတင့္အျဖစ္မ်ားတယ္။ ရိုးရိုးပိုးေသေဆးေပးတာနဲ႔ ေကာင္းတယ္။ ျပန္ျပန္ျဖစ္သူဆိုရင္ ဆီးလမ္းေၾကာင္းမွာ ခ်ိဳ႕ယြင္းခ်က္ ရွိေလမလား စစ္ေဆးရမယ္။

ဆီးလမ္းေၾကာင္းဆိုတာ ဆီးထြက္ျပြန္နဲ႔ ဆီးအိမ္ေတြကို ေအာက္ပိုင္းနဲ႔ ေက်ာက္ကပ္နဲ႔ ဆီးဆင္းျပြန္ေတြကို အထက္ပိုင္းလို႔ ခြဲျခားေလ့လာရတယ္။ ဆီးအိမ္ေရာင္တာကို Cystitis လို႔ေခၚျပီ။ ေက်ာက္ကပ္ေရာင္တာကို Pyelonephritis လို႔ေခၚတယ္။ ဗက္တီးရီးယားကေန အမ်ားဆံုးျဖစ္ေစတယ္။

ကေလးဆီးလမ္းပိုးဝင္ရင္ ဆီးသြားတဲ့အခါ နာလို႔ငိုမယ္။ ဆီးကို အျမန္သြားခ်င္မယ္။ ခဏခဏသြားမယ္။ ဆီးမထိန္းႏိုင္ျဖစ္မယ္။ ေသြးပါေကာင္းပါမယ္။ ကိုယ္ပူတာ၊ ဗိုက္နာတာနဲ႔ အန္တာေတြလည္း တြဲရွိေနတတ္တယ္။

ဆီးလမ္းေၾကာင္းေရာဂါေဆးမ်ား
၁။ Acute Cystitis and Acute Prostatitis ရက္တို က်ား-မ ဆီးလမ္း၊ ဆီးအိမ္ေရာင္ျခင္း၊ က်ား-ဆီးက်ိတ္ေရာင္ျခင္း၊
• Cephalexin 250-500 mg တေန႔ ၄ ၾကိမ္၊ ၁-၃ ရက္၊ ဒါမွမဟုတ္
• Ciprofloxacin 250-500 mg တေန႔ ၂ ၾကိမ္၊ ၁-၃ ရက္၊ ဒါမွမဟုတ္
• Nitrofurantoin 100 mg တေန႔ ၂-၄ ၾကိမ္ (အစာနဲ႔)၊ ၇ ရက္၊ ဒါမွမဟုတ္
• Ofloxacin 200 mg တေန႔ ၂ ၾကိမ္၊ ၁-၃ ရက္၊ ဒါမွမဟုတ္
• Trimethoprim-Sulfamethoxazole 160/800 mg တခါေသာက္ ၁ ျပား၊ တေန႔ ၂ ၾကိမ္၊ ၃ ရက္ေသာက္ပါ။ (တလခြဲေအာက္ကေလး မေပးသင့္။)

၂။ Urethritis ဆီးလမ္းျပြန္အဝ ေရာင္ျခင္း
• Cefixime 400 mg တၾကိမ္။ သို႔ Ciprofloxacin 500 mg တၾကိမ္။ သို႔
• Ceftriaxone 250 mg IM တၾကိမ္။

၃။ Urethritis and Cervicitis caused by Chlamydia လိင္ကတဆင့္ကူးစက္တဲ့ ကယ္လ္မိုင္ဒီယားေၾကာင့္ျဖစ္ေသာ သားအိမ္အဝ-ဆီးျပြန္အဝ ေရာင္ျခင္း
• Azithromycin 1 g ၁ ၾကိမ္။ (သို႔)
• Doxycycline 100 mg တေန႔ ၂ ၾကိမ္၊ ၇ ရက္။ (သို႔)
• Erythromycin base 500 mg တေန႔ ၄ ၾကိမ္၊ ၇ ရက္။ (သို႔)
• Erythromycin ethylsuccinate 800 mg တေန႔ ၄ ၾကိမ္၊ ၇ ရက္။ (သို႔)
• Levofloxacin 500 mg orally တေန႔ ၁ ၾကိမ္၊ ၇ ရက္။ (သို႔)
• Ofloxacin 300 mg တေန႔ ၂ ၾကိမ္၊ ၇ ရက္။

၄။ Acute Pylonephritis ေက်ာက္ကပ္ျပြန္ ရက္တိုေရာင္ျခင္း၊
• Ciprofloxacin 400 mg IV BD or 750 mg တေန႔ ၂ ၾကိမ္၊ (သို႔)
• Levofloxacin 500 mg IV တေန႔ ၁ ၾကိမ္၊ (သို႔)
• Ampicillin 1 Gm တေန႔ ၄ ၾကိမ္၊ + Gentamycin 1 mg/kg TDS IV တေန႔ ၃ ၾကိမ္၊ ၂၁ ရက္။(သို႔)
• Ciprofloxacin 500-750 mg တေန႔ ၂ ၾကိမ္၊ ၂၁ ရက္။ (သို႔)
• Ofloxacin 200-300-400 mg တေန႔ ၂ ၾကိမ္၊ ၂၁ ရက္။ (သို႔)
• Trimethoprim-Sulpfamethoxazole 160/800 mg တေန႔ ၂ ၾကိမ္၊ ၂၁ ရက္။ (တလခြဲေအာက္ကေလး မေပးသင့္။)

၅။ Chronic Prostatitis နာတာရွည္ ဆီးလမ္း၊ က်ား-ဆီးက်ိတ္ေရာင္ျခင္း
• Ciprofloxacin 250-500 mg တေန႔ ၂ ၾကိမ္၊ ၁-၃ လ။
• Ofloxacin 200-400 mg တေန႔ ၂ ၾကိမ္၊ ၁-၃ လ။
• Trimethoprim-Sulfamethoxazole 160/800 mg တေန႔ ၂ ၾကိမ္၊ ၁-၃ လ။ (တလခြဲေအာက္ကေလး မေပးသင့္။)

၆။ Acute Interstitial Nephritis (Penicillins, Cephalosporins, Sulfonamides, NSAIDs, Rifampin, Phenytoin, Allopurinol) ေဆးမ်ားေၾကာင့္ ေက်ာက္ကပ္ေရာင္ျခင္းမ်ား
• Methylprednisolone 0.5-1 Gm/day x ၁-၄ ရက္။ သို႔ Prednisolone 60 mg/day ၁-၂ ပတ္။
