Immunization Day in our country ျမန္မာႏိုင္ငံအမ်ိဳးသားကာကြယ္ေဆးေန႔
Poliovirus ပိုလီယို-ဗိုင္းရပ္စ္၊ Polio (ပိုလီယို) လို႔ေခၚလဲ ရပါတယ္။ Enterovirus အမ်ိဳးအစားျဖစ္တယ္။ ကေလးတိုင္းကို ေမြးကတည္းက ကာကြယ္ေဆး တိုက္ေပးၾကရမယ္။ ပိုးဝင္တာခံရရင္ ျဖစ္စမွာ ကိုယ္နဲနဲပူ၊ လည္ေခ်ာင္းနာ။ မၾကာခင္ Paralysis ေလျဖတ္မယ္။ Meningitis ဦးေႏွာက္ေမွး ေရာင္လာမယ္။ ခုေခတ္မွာ ကာကြယ္ေဆးေၾကာင့္ အေတာ္မ်ားမ်ား (ပိုလီယို) ေရာဂါမျဖစ္ၾကေတာ့ပါ။
ျမန္မာျပည္မွာ ပိုလီယို သူငယ္နာအေၾကာေသေရာဂါ ျဖစ္ပြါးမႈ ၂ ခုေတြ႔ရွိထားတယ္လို႔ ကမာၻ႔က်န္းမာေရးအဖြဲ႔ကေန ၂၁-၁၂-၂ဝ၁၅ ေန႔ကထုတ္ျပန္ပါတယ္။ circulating vaccine-derived poliovirus type 2 (cVDPV2) လို႔ ေခၚပါတယ္။ ၇-၁၁-၂ဝ၁၅ ေန႔က ဓါတ္ခြဲခန္းစစ္ခ်က္အရ သိလာရတာလို႔ဆိုပါတယ္။ တႏွစ္နဲ႔ ေလးလအရြယ္ ေယာက္်ားေလး တေယာက္မွာ acute flaccid paralysis (AFP) ေခၚတဲ့အေၾကာေသေရာဂါကို ၅-၁ဝ-၂ဝ၁၅ ေန႔ကစျဖစ္ေနတယ္။ ကေလးကို အရင္က ပိုလီယိုကာကြယ္ေဆး တိုက္မထားပါ။
၁၆-၄-၂ဝ၁၅ ေန႔ကလည္း ၂ ႏွစ္ ၄ လသားတေယာက္မွာေတြ႔ထားတဲ့ VDPV2 strain အမ်ိဳးအစားပဲျဖစ္တာ ေတြ႔ရတယ္။ ကေလးႏွစ္ေယာက္လံုးက ရခိုင္ျပည္နယ္ကခ်ည္းျဖစ္တယ္။ ပိုးျပန္႔ေနတာ တႏွစ္ေက်ာ္ျပီလို႔ ေကာက္ခ်က္ ခ်ႏိုင္တယ္။ အိမ္နီးခ်င္း ၂၈ အိမ္က ဝမ္းနမူနာကိုလည္း စစ္ေဆးခဲ့တယ္။ ၃ ေယာက္ဆီက ထပ္ေတြ႔ရတယ္။
ျမန္မာျပည္မွာ ၂ဝ၁၄ စာရင္းအရ တျပည္လံုးကာကြယ္ေဆးေပးႏွဳန္း ၇၆% ရွိတယ္။ ေရာဂါအထူးျဖစ္ႏိုင္ေျခ လူဦးေရထဲမွာ ကာကြယ္ေဆးထိုးတာ နည္းတယ္။ ၂ဝ၁၅ ဆာေဗးညႊန္းကိန္းအရေတာ့ (2015 non-polio AFP rate: 1.8; 93% adequate stool collection percentage) ျဖစ္လို႔ေကာင္းပါတယ္။ ျပည္နယ္ေဒသအလိုက္ ကာကြယ္ေပးမႈကေတာ့ ကြာဟေနတယ္။
WHO နဲ႔ Global Polio Eradication Initiative တို႔ကေန အေရးေပၚလုပ္စရာေတြလုပ္တယ္။ ဒီဇဘၤဘာ ၅ ရက္ကေန ၇ ရက္အထိ oral polio vaccine (tOPV) ကာကြယ္ေဆးကို supplementary immunization activity (SIA) အျဖစ္ ၁၅ ျမိဳ႕နယ္မွာ တိုက္ေကြ်းခဲ့တယ္။ ၅ ႏွစ္ေအာက္ကေလးေပါင္း ၃ သိန္း ၆ ေသာင္းရွိတယ္။ ေဖေဖၚဝါရီအကုန္အထိ ရခိုင္ျပည္နယ္ထဲမွာ အဲလို ၃ ေနရာမွာဆက္လုပ္ပါမယ္။
အဲဒီေဒသေတြကို ကာကြယ္ေဆးတိုက္မထားတဲ့ ကေလးေတြ ခရီးမသြားသင့္ပါ။ ေဒသခံေတြလည္း ကာကြယ္ေဆး ထပ္တိုက္တာျဖစ္ျဖစ္ inactivated polio vaccine (IPV) ထိုးေဆးျဖစ္ျဖစ္ရျပီး ၄ ပါတ္ ကေန ၁၂ လအတြင္း ခရီးမသြားသင့္ပါ။

- အထက္ပါ ကာကြယ္ေဆးဇယားကို ျမန္မာႏိုင္ငံအေျခအေနႏွင့္ နီးစပ္သည့္ အိႏၵိယမွာသံုးသည္။ ကာကြယ္ေဆးဇယားသည္ တိုင္းျပည္အလိုက္ ကြဲျပားႏိုင္သည္။
- Pentavalent Vaccine ၅ မ်ိဳး (ဆုံဆုိ႔နာ-ၾကက္ညႇာ-ေခ်ာင္းဆုိး + ေမးခုိင္ + အသည္းေရာင္ (ဘီ) + ေဟမုိဖီးလပ္စ္-ဦးေႏွာက္အေျမႇးေရာင္ + ေဟမုိဖီးလပ္စ္-အဆုတ္ေရာင္) ကာကြယ္ေဆးသစ္ကို သံုးျပီးေနာက္ အထက္ပါဇယားကို အေျပာင္းအလဲလုပ္ရသည္။

• တခါမွမထိုးဘူးေသးတဲ့ကေလး
- ပဌမ = ၁၂-၁၅ လ၊ ဒုတိယ = ၄-၆ ႏွစ္။ (ပဌမအၾကိမ္ေနာက္ ၃ လေနရင္လဲ ေစာထိုးႏိုင္တယ္။)
• ၁၃ ႏွစ္အထက္ကေလးမ်ား
- ပဌမ = ၁၃ ႏွစ္အထက္၊ ဒုတိယ = ေနာက္ ၂၈ ရက္ (တလ) အၾကာ။
- MMRV ကာကြယ္ေဆးထဲမွာ measles ဝက္သက္ + mumps ပသိေရာင္ေရာဂါ + rubella ရူဘဲလား (ဂ်ာမန္ဝက္သက္) + Varicella (chickenpox) ေရေက်ာက္ ကာကြယ္ေဆး ၄ မ်ိဳးပါတယ္။
- Small pox ေက်ာက္ေရာဂါကာကြယ္ေဆး မထိုးေတာ့ပါ။ ကမာၻကေပ်ာက္သြာပါျပီ။ နမူနာခ်န္ထားတာေလးကို ဖ်က္ဆီးျပစ္ဘို႔ ေဆြးေႏြးေနတယ္လို႔ ဖတ္ရေသးတယ္။
- Pentavalent Vaccine (ပင္တာေဗးလင့္-ဗက္ဆင္း) ကာကြယ္ေဆးကို ျမန္မာျပည္မွာလဲ ထိုးေပးေနျပီ။ ကေလးေတြကို ျဖစ္ေစတတ္တဲ့ ကူးစက္ေရာဂါ ငါးမ်ိဳးအတြက္ ေရာထားျပီးသား ကာကြယ္ေဆးျဖစ္ပါတယ္။ ဒိအရင္က ဆံုဆို႔-ၾကက္ညွာ-ေမးခိုင္ေတြအတြက္ DTP သံုးမ်ိဳးတြဲေဆး ဆိုတာလဲ ရွိထားတယ္။ ေနာက္တိုး ၂ မ်ိဳးတိုးလာတာကေတာ့ အသည္းေရာင္ (ဘီ) ကာကြယ္ေဆးနဲ႔ (Hib) Vaccine ျဖစ္တယ္။ တိတိက်က် ေျပာရရင္ (Haemophilus influenza type B) ဗက္တီးရီးယားကေနျဖစ္ေစတဲ့ ဦးေႏွာက္ေျမွးရာင္ေရာဂါ၊ ႏူမိုးနီးယား၊ ေသြးထဲဗက္တီးရီးယားဝင္ျခင္း၊ အေရျပားေရာင္ျခင္း၊ ရိုးတြင္းျခင္ဆီေရာင္ျခင္း၊ အာေခါင္ေရာင္ျခင္း၊ အဆစ္ေရာင္ျခင္းစတဲ့ ကူးစက္ေရာဂါ မွန္သမွ်ေတြကို ကာကြယ္ေပးမွာ ျဖစ္ပါတယ္။
