HIV Prevention ကာကြယ္ေရး

၁။ မဂၤလာပါဆရာ ႕႕႕႕႕ က်ေနာ္အဲ့ညၿပီး ေနာက္ေန႔ညေနမွာပဲ ကူးဆက္ေရာဂါကာကြယ္ေရး ေ၀ဘာဂီကုိသြားၿပီး အက်ိဳးေၾကာင္းေျပာျပၿပီး မကူးဆက္ႏိုင္ေအာင္ကာကြယ္ႏိုင္မလားဆိုေတာ့ ၂၈ ရက္ေသာက္ရမယ့္ ART လား ARV လား အဲ့ေဆးေပးလိုက္ပါတယ္ဆရာ။ အဲ့ေဆးက ဘယ္ေလာက္အထိကာကြယ္ေပးႏိုင္ပါလဲဆရာ။ တဖတ္အမ်ိဳးသမီးမွာ
ေရာဂါ႐ွိမ႐ွိ မသိပါဘူး။ က်ေနာ္အရမ္းစိုးရိမ္မိေနပါတယ္ဆရာ။ ၂ လုံးေသာက္တာ႐ွိတယ္။ ၅လုံးေသာက္တာ႐ွိတယ္ဆိုလို႔ က်ေနာ္ ၅ လုံးေသာက္တာကုိေရြးလိုက္ၿပီး ေသာက္ေနတာ အခုဆို ၄ ရက္႐ွိပါၿပီ။ က်ေနာ္သိခ်င္တာကဆရာ။ အဲ့ေဆးပုံမွန္ အခ်ိန္မွန္ ၂၈ ရက္ေသာက္ၿပီးလို႔ ၃လ ၁ႀကိမ္ေသြးစစ္ ၆လ ၁ႀကိမ္ေသြးစစ္ အဲ့ခ်ိန္မွ HIV မ႐ွိဘူးဆို လုံး၀ စိတ္ခ်လက္ခ် ေနႏိုင္ၿပီလားဆရာ။ ေနာက္မွေရာဂါေပၚလာတာေတြ႐ွိလား။

ကူးဆက္ မဟုတ္။ ကူးစက္လို႔ေရးရ။

၂။ ဆရာ ကြ်န္ေတာ္က HIV လူနာေတြကို ၾကည့္ေပးေနရတဲ့ ဆရာဝန္တေယာက္ပါခင္ဗ်။ ဆရာေရးထားတာတခုမွာ PMTCT အတြက္ အဲဒီမွာ အေမကို AZT combination ေပးတယ္ ကေလးကိုလဲ ေမြးၿပီး တလခြဲအထိ zidivudine ေပးတယ္လို႔ ဖတ္လိုက္ရလို႔ပါ။ National guidelines ေတြက တခုနဲ႔တခု နဲနဲစီကြာလို႔ပါ။ ကြၽန္ေတာ္တို႔သုံးေနတဲ့ WHO Guideline နဲ႔ မတူလို႔ပါ။ ကြၽန္ေတာ္တို႔သုံးေနတာကေတာ့ ကိုယ္ဝန္ေဆာင္မိခင္ ပိုး႐ွိမွန္းသိရင္ Tenofovir/Lamivudine/Efaveranze သုံးမ်ိဳးစပ္ကို ျမန္ႏိုင္သမ်ွျမန္ျမန္ အရင္တိုက္တယ္ဆရာ။ Viral load ကေတာ့ အေျခအေနေပၚမူတည္ၿပီး ေဖာက္တာပါ။ ၿပီးရင္ ေမြးလာတဲ့ကေလးကို nevarapine syrup တိုက္တယ္ ေမြးၿပီး ေျခာက္ပတ္အထိေပါ့။ ၿပီးမွကေလးကို DNA PCR နဲ႔ ေသြးစစ္တယ္ဆရာ။

ကြ်န္ေတာ္အမ်ားဆံုး ကိုးကားႏိုင္တာေတြက ရွာရတာလြယ္တဲ့ ယူအက္စ္နဲ႔ ယူေကက သုေတသနေတြသာ ျဖစ္တယ္။ ေဆးသုေတသနဆိုတလည္း တိုးတက္ေျပာင္းလဲေနတယ္။ ေဆးကုသနည္းေတြရဲ႕ လမ္းညႊန္ခ်က္က ေနရာေဒသအလိုက္ မတူတာလည္း ရွိတယ္။ တိုင္းျပည္ရဲ႕ လိုအပ္ခ်က္၊ တတ္ႏိုင္မႈနဲ႔ ထိေရာက္မႈ စတာေတြကို ၾကည့္ရပါတယ္။ အဲတာေၾကာင့္ ကြ်န္ေတာ္က ျပည္တြင္းမွာ တကယ္ရွိေနတာေတြကို သိပ္သိခ်င္တယ္။ မေန႔ကပဲ တီဘီအေၾကာင္းမွာ ေျဖရေသးတယ္။ ကေလး ကာကြယ္ေဆးထိုးလမ္းညႊန္ကိုသိခ်င္လို႔ ဆရာတေယာက္ကို ေမးထားတာ အဲဒီဆရာက ေမ့ေနျပီနဲ႔တူတယ္။ မပို႔ေသးဘူး။

HIV ကို Pre-exposure Prophylaxis (PrEP) ပိုးနဲ႔ မေတြ႔ခင္ ေဆးၾကိဳတင္ေသာက္နည္းနဲ႔ ကာကြယ္ႏိုင္တယ္။ လိုအပ္လို႔ ေသာက္ရတယ္လို႔ ဆိုလိုပါတယ္။ HIV သံသယရွိတဲ့ လိင္မႈျပဳမယ္ဆိုသူတိုင္း Truvada ေဆး ၂ ျပားကို ၂၄ နာရီ ၾကိဳတင္ ေသာက္ပါ။ တကယ္ပဲ အဲလို အမႈျပဳျပီးရင္ တရက္ ၁ လံုး၊ ၂ ရက္ဆက္ ေသာက္လိုက္ပါ။ ၃ လေနရင္ ေဆးစစ္ပါ။ Truvada ဟာ ေသာက္ေဆးျပားျဖစ္တယ္။ လူၾကီးေတြကို တေန႔မွာ Tenofovir (Viread) 300 mg နဲ႔ Emtricitabine (Emtriva) 200 mg ပါတဲ့ တြဲထားတဲ့ေဆးျဖစ္တယ္။ အစာနဲ႔ နီးနီးေဝးေဝး ေသာက္ႏိုင္တယ္။ ေက်ာက္ကပ္မေကာင္းရင္ ေဆးေလွွ်ာ့ရမယ္။ ေဆးဝါးေဗဒလို Nucleoside analog reverse transcriptase inhibitors (Nukes) ျဖစ္တယ္။ HIV ရဲ႕ RNA ကေန DNA ျဖစ္လာေအာင္ ကူညီေပးတဲ့ (အင္ဇိုင္းမ္) အေျပာင္းအလဲကို သက္ေရာက္ေစတယ္။ ဒီျဖစ္စဥ္ဟာ HIV ပိုးရဲ႕ Genetic code မ်ိဳးဗီဇ (ကုတ္) ကို ကူးစက္ခံရတဲ့ (ဆဲလ္) ရဲ႕ Genetic code မ်ိဳးဗီဇ (ကုတ္) ထဲကို မဝင္ႏိုင္ခင္ လုပ္ေပးတာ ျဖစ္တယ္။

Truvada ေဆးကို ၂ဝဝ၄ ကတည္းက ကူးစက္ခံထားရသူေတြအတြက္ အသိအမွတ္ျပဳထားခဲ့ပါတယ္။ ၁၈ ႏွစ္ေအာက္ ကေလးလူငယ္ေတြနဲ႔ Hepatitis B ဘီပိုးရွိေနသူေတြအတြက္ စမ္းသပ္မႈေတြ မစံုေသးပါ။ ေဆးမေပးခင္ CD4 Cell count နဲ႔ Viral load (ဗိုင္းရပ္စ္) အေရအတြက္၊ ေရာဂါလကၡဏာေတြကိုပါ သိရမယ္။ ဒီေဆးနဲ႔ တျခား (ARVs) ေတြနဲ႔ တြဲေသာက္သူေတြမွာ (ဗိုင္းရပ္စ္) အေရအတြက္ သိသိသာသာ ေလွ်ာ့လာတာတယ္။ CD4 cell counts တက္လာတယ္။

ဘယ္ ART/ARV ေဆးမဆို ေသာက္ခါစမွာ ေခါင္းကိုက္၊ ေသြးဖိအားတက္၊ ေနမေကာင္းသလို ခံစားရတာေတြ ျဖစ္တယ္။ ေဆးေသာက္တာ ၾကာရင္ေတာ့ ခံစားရတာေတြသက္သာလာမယ္။ Truvada ေဆးေၾကာင့္ ေခါင္းကိုက္၊ ပ်ိဳ႕-အန္၊ အနီျပင္ထြက္၊ အစားပ်က္တာေတြ ျဖစ္ေစႏိုင္တယ္။ တခ်ိဳ႕ လူေတြမွာ Creatinine နဲ႔ Transaminases (အင္ဇိုင္းမ္) ေတြတက္တာ ျဖစ္ေစႏိုင္တယ္။ အသဲ၊ ေက်ာက္ကပ္ မေကာင္းရင္ မ်ားေနတယ္။ ရိုးတြင္းျခင္ဆီအားနည္းေစႏိုင္ေသးလို႔ (ကယ္လ္စီယမ္) နဲ႔ ဗီတာမင္ (ဒီ) ေတြပါ ေသာက္တာ ေကာင္းတယ္။ တခ်ိဳ႕မွာ Lactic acid လဲတက္ႏိုင္တယ္။ အသည္းအဆီဖံုးတာ ျဖစ္လာႏိုင္တယ္။ Lamivudine (Epivir, 3TC), Combivir, Trizivir or Epzicom ေဆးေတြနဲ႔ တြဲမေသာက္သင့္ဘူး။

အေမရိကန္ ဖက္ဒရယ္လမ္းညႊန္ကို ၁၄-၅-၂ဝ၁၄ ေန႔က ထုတ္ထားတာပါ။ Truvada ေဆးက ၉၂% ကာကြယ္တယ္။ (ေဂး) ေတြ၊ စအိုလမ္းသံုးသူေတြ ေန႔တိုင္းေသာက္ရတယ္။ ၃ လေနရင္ ေဆးစစ္ပါ။ ေယာက္်ားသံုး (ေလးတက္စ္ ကြန္ဒြမ္) သံုးရင္ ၈၅% ကာကြယ္ႏိုင္တယ္။
