မ်က္စိနာေနတဲ့လူနဲ႔ ၾကည့္ၿပီးစကားေျပာရင္ မ်က္စိနာကူးႏိုင္သလားဆရာ

အဆီအေငၚမတည့္တာေတြ၊ က်ိဳးေၾကာင္းမဆီေလွ်ာ္တာေတြ၊ ယုတၲိမတန္တာေတြ၊ ၾကံဳရာက်ပန္းေတြ၊ မျဖစ္ႏိုင္တာေတြ မၾကာမၾကာေမးလာၾကတယ္။

မ်က္စိနာတာ အမ်ားဆံုးျဖစ္တာက ဗက္တီးရီးယားနဲ႔ ဗိုင္းရပ္စ္ေတြကေနျဖစ္ေစတယ္။ အေအးမိတာကေနလဲ ဆက္ျဖစ္ႏိုင္တယ္။ ဝက္သက္ေပါက္ရာကေနလဲျဖစ္ႏိုင္တယ္။ မိဘေတြဆီကေန ဂႏိုရီးယားပိုး ကူးစက္ရာကေန ျဖစ္တာကဆိုးတယ္။ စာတပုဒ္ေရးထားပါတယ္။ ေရယံုဗိုင္းရပ္စ္ကေနလည္းျဖစ္တယ္။ လိင္ကတဆင့္ကူးစက္တာလည္း ျဖစ္ႏိုင္တယ္။ ေမြးရာပါျဖစ္တာက လိင္ကတဆင့္ကူးစက္ပိုးေတြျဖစ္တဲ့ ကယ္လ္မီဒီးယားနဲ႔ ဂႏိုးရီးယားေတြကေန ျဖစ္ေစႏိုင္တယ္။

တခ်ိဳ႕အခ်ိန္အခါေတြမွာ လူေတြအမ်ားၾကီးမ်က္စိနာၾကာျဖစ္တယ္။ အဲလိုအခါမွာ ၾကည့္တာနဲ႔ကူးတယ္လို႔ တင္စားေျပာတယ္။ အမွန္က ဗိုင္းရပ္စ္ပိုးကူးစက္လို႔ျဖစ္တယ္။ ပိုးက ေနရာတကာမွာရွိေနမယ္။ ေလထဲလည္းရွိမယ္။ မ်က္ႏွာျပင္ေတြ၊ အသံုးအေဆာင္ေတြ၊ လူ႔လက္နဲ႔ ကိုယ္ေပၚမွာရွိေနမယ္။ အဲတာအဝင္ခံရရင္ ကူးမယ္။ လက္ေဆးတာ ေခ်ာင္းဆိုတာအပါအဝင္ တကိုယ္ေရသန္႔ရွင္းေရး အလြန္အေရးၾကီးတယ္။

မ်က္စိနာတာ တဖက္တည္း၊ ႏွစ္ဖက္လံုးျဖစ္ႏိုင္တယ္။ ကူးစက္တယ္။ မ်က္စိပန္းေရာင္ကေန နီရဲေနမယ္။ အေနရခက္မယ္။ ေရာင္ေနမယ္။ နာမယ္။ ၾကည့္မေကာင္းဘူး။ တခ်ိဳ႕ကေဆးမလိုပဲ ေပ်ာက္မယ္။ မရေသးရင္၊ ဆိုးရင္ ပိုးေသေဆး မ်က္စဥ္းခပ္ရမယ္။ ပိုးေၾကာင့္မဟုတ္ပဲ အလာဂ်ီေၾကာင့္လဲျဖစ္ႏိုင္တယ္။ ပန္းေရာင္မ်က္စိဆိုျပီး ေရးထားပါတယ္။ အဲဒီႏွစ္မ်ိဳးေၾကာင့္မဟုတ္ပဲ တခုခုဝင္လို႔ ဥပမာ ေရ၊ ဆပ္ျပာ၊ ေခါင္းေလွ်ာ္ရည္၊ ဖုန္ဝင္လို႔ကေနလည္း ျဖစ္ႏိုင္တယ္။

ကုသဖို႔ရာ တခ်ိဳ႕မွာ ေဆးမလိုတတ္ပါ။ သန္႔ရွင္းေရးလုပ္ေပးပါ။ ဝါဂြမ္းစေလးကို ေရဆြတ္သံုးႏိုင္တယ္။ လက္ကို မၾကာမၾကာေဆးေပးပါ။ လက္ကိုင္ပုဝါ သီးသန္႔သံုးပါ။ မ်က္ကပ္မွန္တပ္ရသူဆိုရင္ ထုတ္ထားပါ။ မ်က္ႏွာသစ္ေပးပါ။ ေရေအးေအးသန္႔သန္႔သံုးပါ။ lubricant eye drops ေခ်ာဆီပါ မ်က္စဥ္းခပ္ပါ။ လက္ကိုင္ပုဝါအစား တစ္ရွဴးေပပါသံုးပါ။ စနစ္တက်စြန္႔ျပစ္ပါ။ မ်က္ကပ္မွန္သံုးသူ ျဖဳတ္ထားပါ။ မ်က္ႏွာသုပ္ပုဝါ မွ်ေဝမသံုးပါနဲ႔။ လက္ခဏခဏေဆးပါ။

ဗက္တီးရီးယားေၾကာင့္ျဖစ္တာအတြက္ အရင္က Chloramphenicol 1% ကလိုရမ္ဖီနီေကာကို အသံုးမ်ားတယ္။ ေကာ္မ်က္စဥ္း အေနနဲ႔လာတယ္။ မ်က္စဥ္းရည္ဆိုရင္ ၂ နာရီ ၁ ၾကိမ္၊ ပဌမ ၂ ရက္နဲ႔ ေနာက္ရက္ေတြမွာ ၄ နာရီ ၁ ၾကိမ္၊ ၄-၅ ရက္ ခပ္ႏိုင္တယ္။ Fusidic acid မ်က္စဥ္းက ကိုယ္ဝန္ေဆာင္ေတြနဲ႔ အသက္ၾကီးသူေတြအတြက္ ပိုသင့္တယ္။ တေန႔ ၂ ၾကိမ္၊ ၇ ရက္ခပ္ပါ။ တျခားပိုးေသေဆးပါမ်က္စဥ္းေတြ Cipro, Gentamycin အမ်ိဳးမ်ိဳးရွိတယ္။

မ်က္စိနာ၊ အလင္းေရာင္ကို မၾကည့္ႏိုင္တာ၊ မ်က္စိမံႈလာတာနဲ႔ နီရဲလြန္းလာတာဆိုရင္ မ်က္စိဆရာဝန္ကိုျပပါ။ မေပ်ာက္ပဲ ဆက္ဆိုးလာရင္ မ်က္ၾကည္လႊာေရာင္တာျဖစ္လာရင္ ဆိုးတယ္။ Keratitis ေခၚတယ္။ မ်က္စိကြယ္ႏိုင္တယ္။

• Allergic Eye diseases အလာဂ်ီေၾကာင့္ မ်က္စိနာျခင္း၊ = Antihistamine + Corticosteroid မ်က္စဥ္းခပ္ပါ။

• Bacterial Keratitis မ်က္စိအၾကည္လႊာေရာင္ျခင္း၊ = Cefazolin 100 mg/ml and Tobamycin 15 mg/ml, Ciprofloxacin 3 mg/ml, Ofloxacin 3 mg/ml မ်က္စဥ္းခပ္ပါ။

• Gonococcal Corneal ulcer ဂႏိုပိုးေၾကာင့္ မ်က္ၾကည္လႊာေရာင္ျခင္း၊ = Ceftriaxone 1-2 Gm IM x 5 days + Erythromycin or Bacitracin E/o မ်က္စဥ္းခပ္ပါ။

• Herpes Simplex Keratitis ေရယံုပိုးေၾကာင့္ မ်က္စိနာျခင္း၊ = Acyclovir 200-400 mg 5 times a day ေဆးတိုက္ပါ။  ေကာ္တီဇုန္းမေပးရပါ။

• Gonococcal Conjunctivitis ဂႏုိးပိုးေၾကာင့္မ်က္စိနာျခင္း၊ = Ceftriaxone 1 Gm IM + Erythromycin or Bacitracin E/o; Chalamydial (Trachoma) = Tetracycline/Erythromycin 250 mg QID Orally or Doxycycline 100 mg BD x 3-5 weeks or Azythromycin 20 mg/kg stat + Surgery for Entropion မ်က္ေတာင္ေမႊးစိုက္ျခင္းအတြက္ ခြဲစိတ္ရန္လိုမယ္။

• Viral Conjunctivitis ဗိုင္းရပ္စ္ေၾကာင့္ မ်က္စိနာျခင္း၊ = Sulfonamide + Prednisolone 0.125% QID မ်က္စဥ္းခပ္ပါ။
